Workers’ Compensation

Workers’ Compensation claims must be authorized by your employer. At the time of your appointment, please provide the following:

Workers’ Compensation claim number
Date of injury
Claim forms
Names, addresses, and telephone numbers of your employer, your immediate supervisor’s name and the Workers’ Compensation insurance carrier

If your claim is denied, you will be responsible for charges as a result of the claim. If you have questions, please contact our billing office staff at 866.263.8525.
