Workers’ Compensation

Workers’ Compensation claims must be authorized by your employer. Prior to your appointment, please provide the following:

We advise all patients to verify Workers’ Compensation coverage with their employer as any and all financial liability ultimately is the patient’s responsibility. For a full list of accepted workers’ compensation networks please contact the office directly. Please note, we do not accept out of state workers’ compensation claims.
