Participating Insurance, Health Plans and Provider Networks
National Spine & Pain Centers participates in numerous insurances, health plans and provider networks. The listing below is intended only as a guide. We advise all patients to verify coverage with their insurance provider as any and all financial liability ultimately is the patient’s responsibility.
Workers’ Compensation
Workers’ Compensation claims must be authorized by your employer. At the time of your appointment, please provide the following:
- Workers’ Compensation claim number
- Date of injury
- Claim forms
- Names, addresses, and telephone numbers of your employer, your immediate supervisor’s name and the Workers’ Compensation insurance carrier
If your claim is denied, you will be responsible for charges as a result of the claim. If you have questions, please contact our billing office staff at 866.263.8525.
