Patient Satisfaction
We always strive to provide our patients with the best possible patient experience. We understand your pain and want your time with us to be as pleasant as possible. If you ever have concerns about some aspect of our services, don’t hesitate to ask one of our knowledgeable staff members or ask to speak directly to the Office Manager. They will all be happy to assist.
Medical Records
You have a right to request a copy of your medical records. Please contact the front desk of the clinic and complete a medical records request form. It usually takes approximately 7-10 days to process the request. There may also be a nominal charge associated with this service.
Late Arrival Policy
Since there is such a high demand for our services, we have to adhere to a strict late arrival policy in order to accommodate each patient in a timely manner. The appointment time you are given is when you are expected to be in the exam room or operating room. We require that new patients come in 30 minutes early, and established patients 15 minutes early to complete paperwork. If you do not arrive 15 minutes early, you may not have enough time to complete the necessary paperwork. Arriving late means, not arriving 15 minutes prior to the appointment time.
Missed Appointment Policy
In order to best serve all of our patients, we require a 24 hour notification should you need to cancel or reschedule your appointment. Should you miss or reschedule your appointment with less than a 24 hour notice, you may be charged a fee that will be due at the time of your next appointment. Insurance companies do not cover fees for missed appointments.
Payment for Medical Services
It is important that you understand you’re assuming financial responsibility for all charges incurred for services rendered. You will be required to pay co-payments, amounts applied to deductibles, and balances of bills not paid in accordance with the benefits of your insurance policy. If you are unable to make payment in full for your medical treatment within 30 days, you must contact the billing office to make payment arrangements.
