We understand that you have probably seen a number of other physicians and specialists before coming to us. So, we want to make your healthcare experience as smooth as possible.
Upon scheduling your first visit with us, our representatives will inform you to bring certain items to your first visit. These include:
Please bring the following with you:
- Photo identification
- Completed New Patient Forms and Signature documents:
- New Patient Questionnaire
- No Fault Form
- Brief Pain Survey 1
- Brief Pain Survey 2
- Worker’s Comp (English)
- Worker’s Comp Form 1 (Spanish)
- Worker’s Comp Form 2 (Spanish
- Health insurance card
- Referral from your referring physician
- It is your responsibility to check with your insurance company to determine if a referral is required.
- Unfortunately, if you do not have your referral YOU MAY NOT BE SEEN.
- X-ray, MRI, any test result, prescriptions
- Or additional information that would be helpful in diagnosing and treating your pain
Kindly arrive at least 10 minutes prior to your scheduled appointment time in order to complete your registration and any other paperwork that may be required. Note that even if you are currently on medication for your pain condition, there is no guarantee that the physician will keep you on that medication.
Important Note
To fully optimize your experience with New York Spine & Pain Physicians, we recommend that you complete all aforementioned documents prior to your first visit. One of our pain management physicians will then perform a complete assessment to determine the best plan of care for you.
